FollowMyHealth is an online tool that UroPartners' utilizes to provide our patients anytime access to their personal health records.
FollowMyHealth allows our patients to:
- review their medical records online in a way that is safe and secure.
- message privately and securely with their providers.
- view results.
- send an appointment request.
- update your health information (allergies, medications, conditions, etc.).
- and much more.
The FollowMyHealth patient portal is available online 24 hours a day, 7 days a week and is accessible with a computer, tablet or smartphone.
Notice: The "Message Your Provider" function will not be available from September 21 - October 5, 2026.
Patients will not be able to send messages to their providers while we are upgrading our Electronic Health Records system. Please call your provider's office with any messages.
